- International Student Application for Admission– Application for Initial and Transfer applicants
- Statement of Understanding Form – Applicant must complete and sign.
- Applicant Affidavit – Financial document completed by the applicant.
- Sponsor Affidavit – Financial document completed by the applicant’s sponsor.
- Transfer Status Form – The applicant will complete section I of the form and have their current college/university’s International Office complete the remaining of the form.
- Third Party Authorization Form – This form allows a third party to receive/submit mail/email for the applicant throughout the application process.
- I-134 Form - Use this form to agree to provide financial support to a beneficiary of certain immigration benefits for the duration of their temporary stay in the United States. *Sponsor from United States use ONLY
