New/Initial Student Checklist
(For students applying from outside the U.S.)

  •  International Student Application for Admission
  • $50.00 Application Processing Fee (nonrefundable)
    • Credit card payments - Contact the Business Office at (817) 598-6265 to submit the payment over the telephone.
  • OfficialHigh School and/or Collegeacademic transcripts for each academic record in the native language with an English translation. Proof of High School equivalence is required through an approved evaluation agency (see the following website for list of agencies http://naces.org/members.html).
  • Copy of passport - If you do not have a passport, provide a copy of your national ID card.
  • Statement of Understanding Form(completed and signed by the applicant).
  • Applicant Affidavit (completed by applicant)
  • Sponsor Affidavit (completed by sponsor)
  • Bank Statements or Letter from sponsor’s financial institution
  • Form I-134 Affidavit ofSupport - required if the student’s sponsor resides in the United States.
  • English proficiency is required for ALL applicants. International students are required to show proficiency by one of the following options:
    • TOEFL Exam – 71 or higher
    • IELTS Exam – Overall Band 5.5 or higher
    • Graduate from an English-speaking country (*See country list below)

*English-speaking countries: American Samoa, Anguilla, Antigua/Barbuda, Australia, Bahamas, Barbados, Belize, Bermuda, British Guyana, Canada(except Quebec), Cayman Islands, Dominica, Falkland Islands, Fiji, Grenada, Guam, Guyana, Ireland, Jamaica/other West Indies, Liberia, Montserrat, New Zealand, Sierra Leone, South Africa, St. Helena, St Kitts & Nevis, St. Lucia, St. Vincent, Trinidad & Tobago, Turks &Caico Isle, United Kingdom, and Virgin Islands, and US Pacific Trust.

  • (Optional) Third Party Authorization Form - allows a third party to receive/submit mail/email for the applicant throughout the application process. (completed and signed by the applicant)

  All of the above requirements must be met prior to the SEVIS I-20 being issued.

  • Proof of a negative TB (tuberculosis) exam. (Skin test or chest X-ray)
  • Proof of the Meningitis Vaccination. Required for applicants 22 years old and younger.

 Transfer Student Checklist

(For students attending a high school/college/university in the U.S. on an F-1 visa)

  • International Student Application for Admission (completed and signed by the applicant).
  • $50.00 Application processing fee (nonrefundable)
    • Credit card payments - Contact the Business Office at (817) 598-6265 to submit the payment over the telephone.
  • Officialtranscripts from ALL colleges/universities attended in and outside the U.S.
    • If you are transferring from an Intensive English program, provide a copy of your IEL certificates or Letter of Enrollment andhigh school transcripts. Proof of High School equivalence is required through an approved evaluation agency (see the following website for list of agencies http://naces.org/members.html).
  • Statement of Understanding Form(completed and signed by the applicant).
  • Applicant Affidavit (completed by applicant)
  • Sponsor Affidavit (completed by sponsor)
  • Bank Statements or Letter from sponsor’s financial institution
  • Form I-134 Affidavit ofSupport - required if the student’s sponsor resides in the United States.
  • Copy of passport page
  • Copy of I-94 printout
  • Copy of visa page
  • Current SEVIS I-20 (first and second pages).
  • Placement Exam Scores – Exam scores must place you into College-Ready courses or co-requisites courses. Acceptable exams: TSI, TSIA2, SAT, or ACT.           
  • Transfer Status Form – Completed by the current college/university’s International Office.
