Health Professions General Student Procedures

Immunization, Screening, and Criminal History Requirements Procedure

All students entering any of the health professions programs must download, sign and return to the the appropriate Allied Health/Nursing department a copy of the Immunization and Screening Requires. The document is available in PDF format and may be downloaded by clicking here.

Allied Health/Nursing students: Students enrolled in Associate Degree Nursing and Vocational Nursing must submit document copies to: Karen Long Office ALHE-130.

Students enrolled in Diagnostic Medical Sonography, Radiological Technology, and Respiratory Care must submit copies to: Tammi Benware ALHE-C. Allied Health/Nursing students may submit faxed copies to: Allied Health (817) 598-6455.

Students enrolled in Emergency Medical Professions courses must submit documentation copies to the EMP office at the Mineral Wells Education Center.

Workforce/Continuing Education students: Must submit document copies to: Workforce-Continuing Education Office STSV Building (Upper Level) at the Weatherford Campus.

DO NOT SUBMIT ORGINIALS — copies will not be made by office personnel. Copiers are located in the Library and Book Store for student use.
